If your cat is experiencing chronic vomiting, diarrhea, or unexplained weight loss, they may be dealing with a chronic gastrointestinal condition. Inflammatory Bowel Disease (IBD) and Intestinal Lymphoma exist along a spectrum, ranging from mild inflammation to intestinal cancer. Here is an overview of why it happens, how we diagnose the underlying causes, and the best paths for long-term treatment.

What Are Feline IBD and Intestinal Lymphoma?

Inflammatory Bowel Disease (IBD) and Intestinal Lymphoma are chronic diseases affecting the gastrointestinal tract. Clinical signs and disease severity can vary greatly between patients. While many cats respond well to treatment, lifelong management and periodic treatment adjustments are often required.

Common Signs to Watch For

If your cat is suffering from chronic GI disease, you might notice:

  • Chronic vomiting or diarrhea
  • Weight loss and poor body condition
  • Decreased appetite or lethargy
  • Nausea (often seen as lip licking)
  • Teeth grinding or excessive swallowing

How We Diagnose IBD and Lymphoma

To determine the severity of the disease and rule out other causes, a combination of diagnostics is recommended:

  • Comprehensive Lab Work & Fecal Testing: Includes a CBC, chemistry panel, urinalysis, and an intestinal parasite screen. (Even if a fecal screen is negative, we often utilize a broad-spectrum dewormer called Panacur).
  • GI Blood Panel (TLI, PLI, Folate, Cobalamin): A specialized blood test that measures intestinal absorption.
  • Abdominal Ultrasound: To rule out abdominal cancers and look for intestinal inflammation.
  • Chest Radiographs & Abdominal Ultrasound: To ensure we check every body cavity (as cancer can hide in the chest).
  • Intestinal Biopsy: This remains the gold standard for officially distinguishing IBD from intestinal lymphoma. This can be done via Surgical Biopsy (exploratory surgery at Cascade Heights) or Endoscopic Biopsy (less invasive, requiring an internal medicine referral).

Treatment Options (Empiric Therapy)

Treatment is typically multimodal. If a biopsy is not pursued, we often use “empiric treatment”—picking a combination of therapies, evaluating the body’s response, and adjusting as needed.

The goal is to feed proteins that the body is drastically less reactive to. Options include:

  • Vitamin B12 (Cobalamin): Most patients with intestinal disease are deficient.
  • Visbiome ® Vet Probiotic: To reset and appropriately adjust the gut microbiome.
  • Welactin® (Omega 3): To reduce systemic inflammation and improve overall intestinal health.

3. Medical Therapy

  • Antibiotics (Tylosin or Metronidazole): Utilized for their anti-inflammatory action in the small intestine and colon.
  • Anti-Nausea/Antacids: Omeprazole to reduce stomach acid and Cerenia® to block acute nausea during flare-ups.
  • Steroids (Prednisolone or Budesonide): Aimed at reducing and controlling intestinal inflammation.
  • Chlorambucil: An immunosuppressive/chemotherapy agent utilized in IBD or lymphoma. It is generally well-tolerated and often has a positive clinical impact.

Prognosis and Follow-Up Care

Prognosis varies with severity and whether the ultimate diagnosis is IBD or lymphoma. Many cats can maintain a good quality of life for years with appropriate management, although periodic flare-ups and treatment adjustments are common.
